HRP Summary Plan Description Supplement and Summary Annual Report

Hawaii Residency Programs, Inc. (HRP) makes medical, dental, disability and life insurance coverage as well as an Employee Assistance Program AND flexible benefits plans available to you and your eligible dependents. You have received benefit booklets and enrollment materials that describe the major provisions of your benefits. This supplement provides additional information you need to know. Together with the benefit booklets you have received, this document serves as the Summary Plan Description required by the Employee Retirement Income Security Act of 1974 (ERISA). Benefits under the HRP plans are provided in accordance with insurance contracts. The contracts are on file with the Plan Administrator. Any participating employee or beneficiary is entitled to examine these materials upon request. In event of a conflict between the Summary Plan Description and the insurance contracts, the insurance contracts will govern. HRP intends to continue the plans indefinitely, but reserves the right to discontinue or change any plan at any time. Termination may be effected by written resolution from the Board of Directors. If HRP terminates any plan for any reason and does not replace the coverage with comparable benefits, you will receive ample notice.
